Whether you are making funeral arrangements for a loved one who has just passed or are preplanning your own funeral the following information will be needed when you meet with us at Scotto Funeral Home.

Arrangements Checklist

Personal Information Checklist Needed for Death Certificate

  • Full Name (including maiden if married) and any variations used on bank accounts or public records
  • Social Security Number
  • Date and Place of Birth
  • Home Address
  • Marital Status
  • Ethnicity
  • Level of Education
  • Occupation (before retirement)
  • Mother’s Name (including maiden name)
  • Father’s Name
  • Person taking responsibility for making funeral arrangements (if not preplanning)
  • Relationship of Responsible Person
  • Address and phone number of Responsible Person
  • Original Cemetery Deed (if burial is selected)

When Making Arrangements Consider the Following Options

  • Do you want a wake at a funeral home?
  • Do you want an open or closed casket for viewing?
  • Do you want the Staff at Scotto Funeral Home to rent another funeral home closer to where you live for the wake?
  • Do you want a Church or religious services?
  • Do you prefer Burial or Cremation?

Additional Items Needed for a Wake

  • Recent Photo
  • Personal effects that you would like in the casket
  • Clothing including undergarments (if personal clothing has not been selected from home, Scotto Funeral Home will be able to provide you with a selection of suites of men and gowns for women as well as undergarments)

Additional Checklist for Cremation

  • What will you do with your loved one’s Cremains?
  • Will you need an urn or other container for the Cremains?
  • Name and Address of individual authorizing cremation
  • An affidavit will be provided to the closest living relative, for them to sign, at the time the arrangements are being made affirming the fact that it was the desire of the deceased to be cremated.

Theses checklists are designed to help you organize important information that will be readily accessible to survivors after the passing of a loved one. They will also help you preplan your funeral when you meet with us and leave a permanent record of these plans so that your family and/or executor will be able to fulfill your wishes.

It is important to not be afraid to talk about death with family members. It is a sensitive topic, but an inevitable part of the life cycle. Discuss these checklists with loved ones; involve them in the planning process.

Discussing your beliefs and desires openly will assure that your final wishes will be carried out and survivors will have the information necessary to make their grief easier to bear.
